This problem with Delivery Plan extensions described blow has been fully resolved. The issue was limited to VSTS services in US regions and affected about 80 accounts total.
The issue was due to a stale cache for the extension and was resolved by clearing the cache and forcing a reload. We believe this was due to timing issues with a recent deployment but need more time to fully investigate root cause. We’ve confirmed that all other regions have the proper extension version in cache and that they are not at risk of experiencing a similar issue.

A potentially customer impacting issue is being investigated. Customers with any ‘Delivery Plans’ Extension installed in their Visual Studio Team Services accounts or trying to install Delivery Plans extension on their VSTS account will receive an error message and will be unable to use this extension. Triage is in progress and we will provide an update with more information.
